For Community Organizations

We are committed to building strong, long-lasting partnerships with community organizations to make sure needed services reach the right people.

As a community organization, you play a critical role in supporting individuals on their journey to better health. It can be challenging to create a seamless network of support. Ensuring the individuals you serve gain access to comprehensive care is important, as is identifying and connecting with the people who need your help the most.

How Wayspring Can Help

We work with health plans and their behavioral health networks to break down the barriers that stand in the way of recovery.

Wayspring’s comprehensive care model actively engages members. Our interdisciplinary care team—including physicians, advanced practice nurses, licensed counselors, and engagement specialists—offers support (virtually and in-person) for physical, behavioral, and substance use care as well as SDoH needs such as housing, mobile phone access, financial assistance, and more.

Addressing everyday life needs is one of the most effective things we do, and typically where we start. SDoH related concerns are the toughest obstacles for lasting recovery.

We are committed to building strong, long-lasting partnerships with community organizations. Through regular check-ins and collaboration, we can ensure that our efforts remain aligned and that our members continue to receive the support they need throughout their recovery journey.
